The answer lies in the simple fact that our foot impression capturing techniques only capture the architecture of the skin. This is fine for most areas, but the metatarsal shafts can be over a quarter inch beneath the surface. To really gain that much needed metatarsal support, we need those soft metatarsal pads to push through the skin some. In this case, a pad is helping us obtain better customized support of the foot.
